For the epileptologist selecting medical, dietary, and surgical therapies for seizures, the diagnosis and management of a patient’s epilepsy is best achieved when the epilepsy etiology is known. Until recently, epilepsy syndromes were generally determined by anatomic studies such as magnetic resonance imaging of the brain and neurophysiological studies with electroencephalography, though next-generation sequencing tests have significantly increased our understanding of the genetic etiologies of children and adults with otherwise idiopathic epilepsy. Genomic studies such as whole genome sequencing have high diagnostic yield, particularly when the patient has both unexplained epilepsy and neurodevelopmental delay.

Furthermore, positive genetic testing often results in the selection of tailored antiepileptic and metabolic/dietary therapies, which can be disease modifying in many cases. Genomic testing can also clarify the risk of recurrence of disease in future children for parents and suggest appropriate surveillance for other organ systems which are comorbidly affected with a positive genetic result. Epileptologists should strongly consider genomic testing when the etiology of a patient’s epilepsy is unclear for additional patient-specific diagnostic and therapeutic guidance.
